Before moving to online registration, many schools asked for an Emergency Card to be completed. Although we do not use that specific terminology in our forms, there are multiple forms that students complete so that we do not ask for the same information twice. If using the system forms for Student Information and Emergency Contacts, then this CSV report would be most relevant to an "Emergency Card." The Student & Emergency Contact Report pulls basic student demographics as well as the emergency contacts from the forms aforementioned.

1. Click the "Generate Report" icon.

2. If you are running the report from the DCP, select which school you would like to run the report for. Skip this step if you are running the report from the SCP. Click Download.
